NCOA and IlluminAge To Co-Sponsor Rehospitalization Webinar

Each year, thousands of older patients are discharged from the hospital, only to be re-admitted. Avoiding preventable re-hospitalizations has become a major cost-savings goal for our health care system. This webinar will look at resources and best practices available at the community level for engaging older patients and their caregivers on the issue of avoiding re-hospitalization.  The webinar is scheduled for April 4, 2013, 1:30 Eastern Time.

The presenter will be Joanne Lynn, M.D.  Dr. Lynn leads the Center on Elder Care and Advanced Illness for Altarum Institute.  A geriatrician, hospice physician, and policy advocate, Dr. Lynn is a member of the Institute of Medicine and of the National Academy of Social Insurance, a fellow of the American Geriatrics Society and The Hastings Center, and a master of the American College of Physicians.  

This webinar will provide a fresh perspective on the challenge of reducing hospital re-admissions, with emphasis on: (1) the importance of educating and empowering older patients and caregivers; (2) the role senior care and aging service professionals can play; and (3) resources available to help at the community level.
